Therefore a management system should be reviewed and updated on a regular basis, however, there are mechanisms within a management system that support you with doing this e.g. management review, internal audits, etc.

Through the completion of this, suitable Specific, Measurable, Achievable, Realistic, and TImely (SMART) corrective actions should be identified which if implemented should prevent any form of recurrence.
Through the completion of this as well as other mechanisms within your Management System e.g. audits, non-conformances, management review, etc. it should allow you to not only correct any raised findings, but also prevent them from reoccurring.
It is important that most corrective actions off the back of an audit have been completed to report this back to the auditor.
